LTSS Service Coordinator - RN Telehealth (Remote)

Position Overview:

The LTSS (Long-Term Services and Supports) Service Coordinator - RN Telehealth is responsible for the comprehensive management of members' cases within the scope of licensure. This role involves performing telephonic clinical assessments, developing and overseeing individualized care plans, and coordinating services to optimize member health across the care continuum. The RN will also supervise and provide direction to non-RN clinicians involved in the member's care.

Responsibilities:

  • Telephonic Clinical Assessments: Conduct assessments to identify, evaluate, and manage members' needs, including physical health, behavioral health, social services, and long-term services and supports.

  • Care Coordination: Identify members at risk for complications and coordinate care in collaboration with the member and the healthcare team.

  • Chronic Illness Management: Manage members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of health benefits.

  • Care Plan Development: Obtain a thorough and accurate member history to develop an individualized care plan, establishing short and long-term goals in collaboration with the member, caregivers, family, natural supports, and physicians.

  • Service Implementation: Assist with the implementation of member care plans by facilitating authorizations/referrals for utilization of services, as appropriate, within benefits structure or through extra-contractual arrangements, as permissible.

  • Interdisciplinary Collaboration: Interface with Medical Directors, Physician Advisors, and/or Inter-Disciplinary Teams on the development of care management treatment plans.

  • Supervision: Direct and/or supervise the work of non-RN clinicians in coordinating services for the member, including assigning appropriate tasks, verifying and interpreting member information, conducting additional assessments as necessary, and reviewing performance regularly.

Requirements:

  • Licensure: Current, unrestricted RN license in New York.

  • Experience: Minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a service coordinator, case management, or similar role.

Preferred Qualifications:

  • Education: Bachelor's degree in Health or Nursing.

  • Language Skills: Bilingual proficiency in Spanish, Mandarin, Cantonese, Russian, Korean, Urdu, or other languages is highly preferred.
